If you're considering Brookdale Fort Collins for a Medicaid or PACE family member, do more research first. My father was a PACE participant through InnovAge, and despite his monthly obligation being fixed under his enrollment paperwork, Brookdale billed him at full private-pay rates for months, then billed him again for personal care services PACE already covers. We spent almost a year chasing corrections, at one point told by corporate billing that internal notes showed a plan to discharge him during an active state billing investigation.
Colorado's Department of Health Care Policy & Financing formally investigated and found that Brookdale failed to accept PACE payment as payment in full and attempted to bill for services already covered. That's not my opinion, that's the state's own finding.
Even after that, Brookdale issued a credit in July, approved by their own Executive Director, then quietly reversed it two months later and rebilled us the same amount with zero explanation. If you need a facility that bills accurately and treats Medicaid residents fairly, look elsewhere. This whole experience has been and continues to be exhausting, insane, a massive time suck, and stressful in the extreme. I cannot believe how blatantly they seem to disregard patient's rights, state and federal laws and billing rules, the consumer protection act, and basic human decency. I can only imagine they are doing this to other residents there, who may not have the time or resources or ability to fight these practices.
On the bright side, there were several very good and kind care givers there.
My Mother recently passed. She lived in Clarebridge Brookdale Fort Collins. The day she moved into Clarebridge we all felt welcomed. Everytime I walked into the door, I was welcomed with a smile. The atmosphere was calming. The residents all seemed content. My mom was always dressed in clean clothes and her room was organized and clean. They comforted my mom when she passed and comforted me when i arrived soon after, in fact they are still supporting me thru all the moving out process. I am thankful my mom was in Clarebridge at Brookdale.
We recently put my Mom in Brookdale and have had a great experience. Love the staff, the care they provide is topnotch. We looked at a lot of places in Fort Collins and this one met my Mom's needs the best.
Our family’s experience with Brookdale Fort Collins spans about 2½ years, as they cared for my mother-in-law during the later stages of her dementia until she recently passed away.
Before moving her into memory care, my wife and I tried everything we could to care for her ourselves. We even moved her into our home. It quickly became clear that we were not equipped to provide the 24/7 care she needed while managing careers and three kids in college. After evaluating several facilities, we chose Brookdale.
The transition was not easy. Dementia is hard on everyone, and my mother-in-law was very upset about the move at first. There were difficult phone calls and plenty of guilt on our end. Looking back now, it was absolutely the right decision. We simply needed help, and Brookdale provided it.
The staff kept her safe and cared for her with compassion all the way to the end. They communicated well with our family, especially whenever something happened, such as a fall or health change.
For families going through this decision: be prepared for an adjustment period—for both your loved one and for you. It’s emotionally difficult, and the guilt can be heavy. But once my mother-in-law settled in, it became clear that Brookdale was the best place for her.
She ended up happier there than she had been living with us. She had her own space, her cat, friends, and activities. She loved walking the halls. We visited at least once a week, and it became a place where we could simply spend time together instead of constantly worrying about her safety.
We are truly grateful to the Brookdale staff. Many of them formed real bonds with her and clearly cared about her wellbeing. Even at the end of her life, they treated our family with kindness and compassion. One employee even adopted her cat.
We would also like to thank Bloom Hospice Care for the support they provided during her final months.
This work is not easy, and we deeply appreciate the people who do it.
